Motor Claim Process
Step 1: Complete the Claims Form
Complete the appropriate form(s) depending on the claim(s) you wish to make. Different forms need to be completed for claims against riders, death, group term insurance and gratuity. All details with respect to the client’s ID, policy number, policyholder’s name, etc. will also be required.
Step 2: Arrange for Appropriate Documents
Based on the claims made, the appropriate documents need to be provided. The documents can be submitted as original or photocopies, attest by authorised persons.
Step 3: Submit the Required Documents with the Claims Form
Once the form is completed and documents are in place, submit them to the nearest branch office. Alternatively, you can also send us your documents by post or fax to: +255 22 2138852
Payment of excess amount
Our representative will contact you with the applicable excess amount within 1 working day.
Ways of Registering a Claim

Documents Required
In the event of death, please avail the following documents not more than 1 year after the incident:
a . The policy contract
b . A fully completed claim form
c . Proof of death by providing at least one of the following documents
- Death certificate
- Police report if death is accidental (PF 90)
- Burial Permit (from a registered medical institution)
- Burial Permit (from a Ward Executive Officer, not any other office)
d . Proof of Age of the Deceased by providing at least one of the following documents:
- Birth Certificate
- Passport
- Voters Card
- Sworn affidavit (from an Advocate of the High Court or a District Magistrates Court or a Resident Magistrates Court or a Higher Court but not any Lower Court)
e . Proof of Identity of payee by way of any one of the following:
- Passport
- Employers Identity Card
- Voters Card
- Bank Identity Card
- Letter of Identity from a Ward Executive Officer, not any other officer
Non-Motor Claims
Accidents / Losses are unfortunate realities of life and in no time, it risks your hard-earned assets / properties / businesses / Cash and could easily reduce to nothing. While we cannot take away the pain, but we can commit ourselves to make these losses good quickly and efficiently with minimal documentation requirements & expedited services.
Documents Required
All the claims related to the above takaful products warrants following basic claim documentation for processing the claims which shall include but not limited to the following:
- Claim intimation Note – by Verbal/Email/Fax
- Duly Filled Claim form – Blank Form available on website/office
- Evidence supporting Nature & Extent of loss/damage
- Photographs depicting the loss/damage
- Discharge voucher-cum-Letter of subrogation duly executed at the time of settlement
